Our Post-Acute Care Centers provide inpatient nursing care and services for individuals in need of round-the-clock nursing care. Our centers are licensed by the states in which they operate and are regulated by state and federal laws and regulations. Nurses (with nursing aides and assistants), therapists and social workers provide care and service to residents at our Post-Acute Centers.
Agapé Senior provides short-term express rehabilitation, inpatient hospice, and long-term care at its state-of-the-art Medicare-certified facilities. Our caring, compassionate and highly-trained healthcare professionals are focused on the needs of the residents, and are committed to exceptional care around the clock. Our skilled nursing team is committed to providing superior medical care with a professional yet personal demeanor. Our nurses, CNA’s, and therapists closely follow plans of care provided by referring physicians. Their efforts are supported and enhanced by a full complement of therapeutic and rehabilitative programs, all placing emphasis on preserving or improving quality of life.
Agapé Senior Express Rehabilitation Program
Agapé Senior provides short-term rehabilitation at its state-of-the-art Medicare-certified facilities. Our caring, compassionate and highly-trained healthcare professionals are focused on the needs of patients.The typical length of stay for a rehabilitation patient is 28 to 30 days. Significantly shorter stays are the norm for certain procedures; the goal for knee replacement rehabilitation is 8 days.
Our rehabilitation team is committed to providing superior medical care with a professional yet personal demeanor. Our nurses, CNA’s and therapists closely follow plans of care provided by referring physicians. Their efforts are supported and enhanced by
a full complement of therapeutic and rehabilitative programs, all placing emphasis on preserving or improving quality of life.
In addition, when a patient has a medical condition that requires a hospitalization it is important to remember that during illness not only does the body become affected by the disease but also by the deconditioning that comes from the simple fact that the patient has been bed-ridden. The Transitional Rehabilitation Center offers the opportunity to have a period of time where patients are still receiving nursing care and therapy services in order to get them to their optimum level of independence and successful transitions.
Agape Therapy: Get Better Faster
The primary focus of Agapé Senior Post-Acute Care Centers are to help patients return home as quickly as possible. Physical, occupational, and speech therapies are offered 7 days a week. Studies show that intensive, concentrated and interdependent therapy sessions can greatly reduce the amount of time needed for recovery. Physical therapy is a type of rehabilitative health care that utilizes exercise and equipment to assist individuals in the development or recovery of muscular strength, flexibility, and range of motion.
Like physical therapy, occupational therapy involves physical movement and activity. Occupational therapy consists of a wide variety of activities that are designed to increase a person’s capabilities and functionalities in their daily lives: activities such as eating, grooming, and dressing. Speech therapy is the diagnosis and treatment of speech disorders. Speech therapy provides patients with treatments that increase abilities with swallowing, communication, and cognition.
If a therapy patient is discharged from Agapé to the home, our therapy program can provide a seamless transition by having the inpatient therapist transition to serving as the outpatient therapist.

Hospice In-Patient Suites: Competent and Committed to Your Comfort
Agapé Senior’s Hospice In-Patient Suites are a direct result of our commitment. The Suites are a dedicated unit within each of our Post-Acute care Centers in Conway, Rock Hill and West Columbia. Each facility has the 24-hour RN coverage necessary for intense nursing intervention and monitoring. Our staff has extensive experience in hospice and palliative care, focusing on comfort care verses restorative care. We realize that optimum physical comfort for patients is achieved through a combination of approaches: pain management, medication management expertise, palliative therapy and requisite medical equipment and supplies. Our physicians and nurse practitioners are actively involved in the direction and supervision of the care given to each and every patient. Consultation that is available and accessible 24 hours a day ensures proactive care.
Not Just Patient Care, It is Family Care.
At Agapé Senior, we are aware that the conditions and wishes for every patient are unique. Each of our In-Patient Suites offers a social worker who is devoted to the needs of the patient receiving hospice and palliative care, as well to the needs of the patients' family members. Each social worker is well-versed in addressing the psychosocial and emotional needs of patients and family members, and has quick access to in-house or community support services.
Agapé Senior also recognizes the importance of meeting the spiritual needs of patients and families. Our chaplains are accessible for patients and families as they deal with the difficult reality of end-of-life or chronic-illness care.
