Agapé Senior Express Rehabilitation Program
After hospitalization, patients must recover from disease or injury. In addition, they must recover from the deconditioning that accompanies inactivity. Many individuals need intense rehabilitative therapy in a skilled nursing setting. We offer a full complement of individualized treatments in physical, speech and occupational therapies which are available seven days a week. Agapé’s therapy gym offers the latest in equipment and technology for rehabilitation. Our nurses, CNA’s and therapists closely follow plans of care provided by referring physicians. Our goal is to get you to your optimum level of function faster.
When the time comes to return home, our staff works with patients and their families to plan a successful transition. An Agapé therapist may choose to assess the home environment and make recommendations on safety issues or helpful adaptations that may be made to their home environment.
Our patients have the option of continuing their relationship with the physicians and therapists that were such a crucial part of their recovery. The Agapé Senior Primary & Palliative Care office is conveniently located on Sunset Boulevard in West Columbia, and is happy to accept new patients. 
In addition, Agapé therapy also has an outpatient clinic that is open to the public at the same location. Patients find it comforting to keep the same physician and therapist after the transition home.
If an individual is not quite ready to return home, the Agapé Senior - West Columbia campus offers living options from independent living to enhanced care assisted living. The Village, Agapé Senior’s senior patio-home community is a short drive down Leaphart Road from the West Columbia campus.
Agapé Hospice In-Patient Suites
Agapé Senior is proud to offer the 44-bed Hospice and Palliative In-Patient Suites where general inpatient hospice (GIP) and palliative medical care are provided. Agapé Senior Long-term Care
If returning home or moving to assisted living are not viable options, we also offer long-term nursing care. Our experienced staff of nurses and CNAs offers round-the-clock care to meet the needs of each patient. Our Agapé Senior Primary & Palliative Care physicians and staff provide assistance with managing symptoms of chronic health issues and tackle any new medical issues that may arise.
Agapé Senior Respite Care
Respite Care is provided when caregivers may need to arrange temporary nursing care for their loved one. Caregivers may choose to use respite during events like vacations or caregiver illness.
