alliative care is a type of health care that is designed to provide relief from the symptoms of major illnesses; pain management is the most common type of palliative care. Agapé Senior Post-Acute Care Centers provides palliative care for hospice patients or other individuals who require pain/symptom management. Education, communication and support for palliative care patients and their families are also provided. Agapé Senior appreciates the importance of a personal touch. Transitioning from rehabilitation to the home or an assisted living facility regularly requires a carefully considered interim care plan. Our medical staff works with discharged patients and their families to develop individualized treatment plans. Such a plan includes healthcare, but also social services and tailored dietary guidelines. When and if a period of transition is needed, our long-term care facilities and short-term rehabilitation programs are always available.

The Suites: Competent and Committed to Your Comfort

Agapé Senior’s Hospice and Palliative In-Patient Suites are a direct result of commitment. The Suites are a dedicated unit within each of our Post Acute Care Centers in Conway, Rock Hill and West Columbia. Each facility has 24-hour RN coverage providing intense level of care. Our Suites staff has extensive experience in hospice and palliative care, focusing on comfort care through the use of medical, social and spiritual means. We achieve optimum comfort for patients through a combination of approaches: medication management, palliative therapy, spiritual counseling and psychosocial counseling. Our physicians and nurse practitioners are dedicated to the care given to each and every patient. Their consulting services are available and accessible 24 hours a day promoting immediate response to the needs of the patient.

Family Care, Not Just Patient Care

Agapé Senior is aware that the conditions and wishes of every patient are unique. Each of our In-Patient Suites offers a social worker and chaplain who are devoted to the needs of the patients receiving hospice and palliative care, as well as their family members. Each team member is well-versed in addressing the emotional needs of patients and family members, and has access to in-house or community support services.

Agapé Senior focuses on meeting the physical and spiritual needs of patients. Our chaplains are accessible for patients and their families as they deal with the difficult reality of chronic illness or end-of-life care.
